Appointments

Appointments and Office Hours 

Our clinic hours are Monday through Friday from 8:00 a.m. to 5:00 p.m. Consultations with our doctors and audiologists are available based on current appointment openings, except in emergency situations.

Emergencies

In the case of an emergency, please seek medical care in the emergency room. For urgent care needs, our patients may call (320) 252-0233 or (800) 450-3223, 24 hours a day. If you are not our patient, urgent care should be sought from your primary care provider or the emergency room.

Medications

Please bring a complete list of all current medications and prescriptions to your appointment.

Insurance

We participate with most insurance companies. Please check with your insurance company to find out if your plan requires you to obtain prior authorization before coming to the clinic. Bring your current insurance card(s) with you.

Credit Policy

For your information, our credit and payment policy is as follows:

• All co-pays are due the date of service.
• For surgical charges, your remaining balance will be due in full following the insurance payment.
• Our patient accounts manager is available to assist you with other financial arrangements.
• Need to pay your bill or hearing aid over time? Here are some financing solutions.

This form is for your use in releasing information from our clinic to the medical provider of your choice.

Smoking

We are a smoke-free environment and do not allow smoking in our clinic or on clinic grounds.
